Image seems to be a advertisement for Scalf's Indian River tonic - the Master Medicine.  Bottle states for men Women and children vegetable compound. Underneath the rural scene including a river with and Indian with feathers about his head paddling down in a canoe - are the words: New Scalf Medicine Co., Harlan, KY

Believe that the artist that drew the original image, was Joe Vowell - as his name is clearly beneath it
